Sciatica is a symptom, not a diagnosis. The word describes pain, numbness, or weakness that follows the path of the sciatic nerve from the lower spine through the glute, down the back of the thigh, into the calf and foot. According to the NIH National Institute of Neurological Disorders and Stroke, the cause is almost always compression or irritation of one of the lumbar or sacral nerve roots. Find the source of the compression, address it, and most cases settle within a few weeks.

I am Dr. Jason Forward, DC. I run Vanguard Chiropractic in Brookside. This page is the short, practical version of what we do for sciatica here. For the longer mechanism breakdown, see the complete sciatica guide on the blog.

How chiropractic care addresses mechanical sciatica

Three steps. Find the cause, restore motion, rebuild support. The order matters.

1

Find the source

Range-of-motion testing, straight leg raise, slump test, palpation of the lumbar spine, sacroiliac joints, and piriformis. Identify which nerve root, at which level, compressed by which structure.

2

Restore motion

Specific, controlled adjustment to the restricted segment. For disc-driven sciatica, decompression-style mobilization and modified positioning. The goal is to take pressure off the nerve at the actual point of compression.

3

Rebuild support

Five to ten minutes a day of specific work, glute activation, hip-hinge mechanics, anti-rotation. Whatever the exam revealed as deficient. The goal is durable, not heroic.

The four mechanical sources of sciatica

Sciatica is the symptom. The exam answers which of these is the source. Each one has a different point of compression, a different exam signature, and a different treatment plan.

Cause 1

Lumbar disc herniation

Disc material extrudes posteriorly into the lateral recess and compresses the L4, L5, or S1 nerve root. The most common identifiable cause of true sciatica.

Pain pattern: low back into glute, down the back of the thigh, often into the calf or foot.

Cause 2

Lumbar spinal stenosis

Narrowing of the central canal or lateral recess pinches the nerve at one or more levels. Tends to develop slowly and worsen with prolonged standing or walking.

Pain pattern: bilateral leg ache that eases when leaning forward or sitting down.

Cause 3

Piriformis syndrome

The piriformis muscle in the deep glute compresses the sciatic nerve at the greater sciatic notch. Often missed because the pain mimics disc-driven sciatica almost exactly.

Pain pattern: deep buttock pain, often worse with sitting on hard surfaces or driving.

Cause 4

Sacroiliac joint dysfunction

A restricted or hypermobile SI joint refers pain along the same path as true sciatica. Often a contributor when the pain pattern stops at the knee instead of the foot.

Pain pattern: low back and posterior thigh, rarely past the knee, worse with single-leg stance.

What your first sciatica visit looks like, step by step

Most patients overestimate how long this takes. Here is the actual sequence, in order, the first time you walk in.

0 min

Walk in, sign in

Two pages of intake. What hurts, when it started, what makes it worse, what you have already tried, any red-flag history. No insurance card, no driver's license required.

3 min

Brief intake conversation

I read your form, then ask you to tell me the story in your own words. Often the most useful detail is the one that did not fit on the form.

6 min

Focused sciatica exam

Range of motion, straight leg raise, slump test, palpation of the lumbar spine, sacroiliac joints, piriformis. If your history or exam findings suggest a disc lesion or stenosis that needs imaging, I will refer you out for that and tell you straight up.

10 min

The adjustment

Specific, focused, board-safe. For mechanical sciatica, the work is on the lumbar segments, sacroiliac joints, or piriformis depending on what the exam showed. You hear cavitations on most segments. Most patients feel some immediate relief, although that varies.

15 min

Pay $40, walk out

Cash, card, HSA, or FSA at the front desk. Same total every visit. I will tell you whether to come back tomorrow, next week, or only if it flares again. No pressure to schedule.

Why sciatica keeps coming back, and what actually breaks the cycle

The most common reason sciatica recurs is not bad luck. It is mechanical. The pain settles when the irritation calms, but the underlying compression at the nerve root never resolved. Six weeks later, six months later, the same load patterns build the same problem again.

Mayo Clinic notes that conservative care, including physical therapy and manual treatment, is appropriate first-line for most cases of sciatica. The American College of Physicians, in its 2017 clinical practice guideline, recommends spinal manipulation as a first-line non-pharmacologic option for low back pain with or without leg symptoms. The recommendation is based on a moderate-quality body of evidence showing reduced pain and improved function compared to usual care.

What a sciatica treatment plan actually looks like

Most acute mechanical sciatica resolves in four to six visits across two to four weeks. The arc below is the typical pattern. Yours may move faster or slower depending on the exam findings.

Day 1 · The Bandaid

The bandaid decision

Today is the bandaid. Walk in, exam, first adjustment, walk out. If a bandaid is all you want, that is what you get, no argument. Not my favorite outcome, but it is your call. Same $40, no pressure to come back.

Relief Care · Weeks 1-3

True recovery

True recovery needs frequency: 2-3 visits per week for 2-3 weeks. The tissue calms, the segment regains motion, the nerve quiets. Many people stop here when they feel good. The issue often comes back.

Corrective Care · Weeks 4-8+

Improvement

1-2 visits per week, weeks 4 through 8 or so. The chronic guarding pattern releases, supporting mechanics rebuild, the segment holds its motion between visits. Some people stop here. The issue often comes back.

Wellness Care · Months-Years

Optimization

The rare patient. Ongoing scheduled care regardless of how you are feeling, so the flare-and-recover cycle never starts. Months to years of prioritized, dedicated chiropractic. This is the real gold and the preferred course. Same $40 every visit, no contracts, stop any time.

When sciatica is not a chiropractic case

Most sciatica is mechanical and responds well to conservative care. Some cases need medical evaluation first.

Walk into an emergency room, not Vanguard Chiropractic, for any of these:

  • Sudden loss of bowel or bladder control
  • Numbness in the saddle area between the legs
  • Rapidly progressive weakness in one or both legs (cannot lift your foot, cannot stand on tiptoe)
  • Sciatica following a significant trauma like a fall from height or a motor vehicle accident
  • Severe night pain with unexplained weight loss or fever

These can signal cauda equina syndrome, a fracture, a tumor, or an infection. They are emergencies. They need imaging and medical evaluation immediately.

The rest of recurring or acute mechanical sciatica is, in my experience, treatable. The screening matters every single time, which is why every first visit at Vanguard Chiropractic includes a focused exam before any adjustment.

Frequently asked questions

Is sciatica a chiropractic case or a medical case?
Most sciatica is mechanical and responds well to chiropractic care. Cases that need medical workup first include any of the red flags above (saddle numbness, bowel or bladder loss, rapid weakness, post-trauma onset, fever or unexplained weight loss). The screening at your first visit will sort that out.
How long does sciatica take to resolve with chiropractic care?
Most acute mechanical sciatica responds to conservative care within four to six weeks, sometimes faster. Recurring sciatica often responds in a similar window once the mechanical cause is identified, plus a longer arc of home-work to make the next flare-up less likely.
Do I need an MRI before I come in?
Usually no. For most cases of mechanical sciatica without red flags, the exam tells us what we need to know to start. If your history or exam suggests a disc lesion that would change the treatment approach, I will refer you for imaging then.
Do you take insurance for sciatica visits?
No. Vanguard Chiropractic is cash-pay only. Every visit is $40 flat, paid at the desk. We do not bill insurance and we do not work with personal-injury attorneys or auto-injury claims.
What if my sciatica was caused by a car accident?
Vanguard Chiropractic is not a personal-injury or auto-injury clinic. If your sciatica is part of an active PI or auto-injury claim, you will be better served by a clinic that handles that documentation. Several Tulsa clinics specialize in PI cases. We can still see you on a cash-pay basis if you prefer that, but we will not bill any insurance or attorney.
How many visits will I need for recurring sciatica?
Most acute cases settle in four to six visits over a few weeks. For recurring sciatica, I recommend once-a-week visits for at least twelve weeks after the initial phase, then we taper as the case stabilizes. Each flare can compound the underlying nerve irritation, so prevention is the responsible approach. Same $40 every visit. No contracts, no packages, no membership fees, and you can stop any time.
Why do my hamstring stretches help my sciatica only temporarily?
Stretching can offload the nerve temporarily but does not restore segmental motion to a restricted vertebra, take pressure off a bulging disc, or change pelvic torsion. The mechanical cause remains, and load builds again until the next flare. Stretching is useful as part of a plan, not as the plan itself.
What does a $40 sciatica visit actually include?
Intake, focused exam, the adjustment if your case is appropriate for chiropractic, and a brief home-work conversation. Same flat $40, every visit, every patient. No upsells, no packages, no sticker shock at the desk.
