Most disc herniations get better with conservative care. According to Mayo Clinic, the majority of people with a herniated disc improve within a few weeks to months without surgery. The American College of Physicians, in its 2017 clinical practice guideline, recommends spinal manipulation as a first-line non-pharmacologic option for low back pain with or without leg symptoms. Surgery is reserved for cases that fail conservative care, have severe progressive neurological deficit, or show red-flag findings.

I am Dr. Jason Forward, DC. I run Vanguard Chiropractic in Brookside. Disc herniation is one of the most common reasons people walk in. This page is the short, practical version of how I work it up. For the longer story on imaging, surgery thresholds, and what most people get wrong about disc problems, see the bulging or herniated disc deep-dive on the blog.

How chiropractic care addresses a symptomatic disc

Three steps. Find the lesion, reduce nerve irritation, rebuild support. The order matters.

1

Find the lesion

Targeted exam: dermatomes, motor strength, deep tendon reflexes, straight leg raise, slump test, foraminal compression. Identify which nerve root, which level, with or without true compression. Decide if imaging changes the plan.

2

Reduce irritation

For an acutely symptomatic disc, the work is decompression-style mobilization, modified positioning, gentle traction, and avoiding the loads that flare it. High-velocity manipulation is selected, not default. The goal is to take pressure off the nerve.

3

Rebuild support

Spine-sparing movement patterns, hip-hinge mechanics, anti-rotation, daily five to ten minutes of specific work. Whatever the exam revealed as deficient. The goal is durable, not heroic.

The four common disc-lesion scenarios

Disc problems are a spectrum, not a single diagnosis. The exam answers which scenario you are in. Each one has a different prognosis and a different treatment arc.

Scenario 1

Disc bulge without nerve compression

The annulus is weakened and the disc shape is altered, but the nerve root has not been compressed. Often pain-free or mild back pain only. Conservative care is highly effective.

Pattern: low back stiffness or ache, no leg symptoms, normal motor and sensory exam.

Scenario 2

Herniation with radiculopathy

The annulus is breached and disc material compresses or chemically irritates a nerve root. The most common reason people walk in. Conservative care works for most cases over four to twelve weeks.

Pattern: low back pain plus leg pain following a dermatomal path, often worse with flexion or sitting.

Scenario 3

Sequestered or large disc fragment

A fragment of disc material breaks free into the spinal canal. Less common but can cause significant compression. Imaging is usually warranted; conservative care still useful for many cases.

Pattern: severe radicular pain, often with motor weakness or sensory loss; needs MRI to map.

Scenario 4

Cervical disc herniation

Same biology, different segment. The C5-C6 and C6-C7 levels are the most common sites for cervical disc herniation. Treatment principles are the same: confirm the level, reduce irritation, restore mechanics.

Pattern: neck pain plus arm pain following a dermatome, often with grip weakness or numbness in specific fingers.

What your first herniated-disc visit looks like, step by step

Most patients overestimate how long this takes. Here is the actual sequence, in order, the first time you walk in.

0 min

Walk in, sign in

Two pages of intake. What hurts, when it started, what makes it worse, what you have already tried, any red-flag history. No insurance card, no driver's license required.

3 min

Brief intake conversation

I read your form, then ask you to tell me the story in your own words. Often the most useful detail is the one that did not fit on the form.

6 min

Focused disc-and-radiculopathy exam

Range of motion, dermatomes, motor strength testing, deep tendon reflexes, straight leg raise, slump test, foraminal compression. If your exam suggests a lesion that needs imaging or surgical opinion, I will tell you straight up and refer you out.

10 min

Conservative care, if appropriate

Specific, focused, board-safe. For acutely symptomatic disc cases, the work is decompression-style mobilization and modified positioning. High-velocity manipulation is selected when the exam supports it, not as a default.

15 min

Pay $40, walk out

Cash, card, HSA, or FSA at the front desk. Same total every visit. I will tell you whether to come back tomorrow, next week, or to get an MRI first. No pressure to schedule.

Conservative care first: what the guidelines actually say

The first-line recommendation for most disc herniations is conservative care for at least four to six weeks before considering surgery, unless red-flag findings change the calculus. The American College of Physicians 2017 guideline recommends spinal manipulation as a first-line non-pharmacologic option for low back pain with or without leg symptoms. Mayo Clinic echoes this: most herniations improve without surgery, and conservative care reduces pain and improves function for the majority of patients.

The exception is the case with progressive motor loss, cauda equina findings, or severe pain refractory to several weeks of conservative care. Those are the cases where a surgical opinion changes outcomes. The exam at your first visit sorts you into the right category.

What a herniated-disc treatment plan actually looks like

Most acute disc-driven radiculopathy responds to conservative care across four to twelve weeks. The arc below is the typical pattern. Yours may move faster or slower depending on the exam and imaging findings.

Day 1 · The Bandaid

The bandaid decision

Today is the bandaid. Walk in, exam, first adjustment, walk out. If a bandaid is all you want, that is what you get, no argument. Not my favorite outcome, but it is your call. Same $40, no pressure to come back.

Relief Care · Weeks 1-3

True recovery

True recovery needs frequency: 2-3 visits per week for 2-3 weeks. The tissue calms, the segment regains motion, the nerve quiets. Many people stop here when they feel good. The issue often comes back.

Corrective Care · Weeks 4-8+

Improvement

1-2 visits per week, weeks 4 through 8 or so. The chronic guarding pattern releases, supporting mechanics rebuild, the segment holds its motion between visits. Some people stop here. The issue often comes back.

Wellness Care · Months-Years

Optimization

The rare patient. Ongoing scheduled care regardless of how you are feeling, so the flare-and-recover cycle never starts. Months to years of prioritized, dedicated chiropractic. This is the real gold and the preferred course. Same $40 every visit, no contracts, stop any time.

When a herniated disc is not a chiropractic case

Most disc herniations respond well to conservative care. Some cases need medical or surgical evaluation first.

Walk into an emergency room, not Vanguard Chiropractic, for any of these:

  • Sudden loss of bowel or bladder control
  • Numbness in the saddle area between the legs
  • Rapidly progressive weakness in one or both legs (cannot lift your foot, cannot stand on tiptoe)
  • Disc symptoms following a significant trauma like a fall from height or a motor vehicle accident
  • Severe night pain with unexplained weight loss or fever
  • Sudden severe weakness in the arm or hand with cervical disc symptoms

These can signal cauda equina syndrome, a severe sequestration, a fracture, a tumor, or an infection. They are emergencies. They need imaging and medical evaluation immediately.

The rest of mechanical disc-driven pain is, in my experience, treatable. The screening matters every single time, which is why every first visit at Vanguard Chiropractic includes a focused exam before any care.

After the Pain Goes Away: Why Ongoing Care Beats Crisis Care

If your herniated disc has come back more than once, the mechanical pattern that caused it is still there. Treating only when it flares means treating the same problem repeatedly while the underlying restriction compounds. The honest clinical recommendation for recurring herniated disc is ongoing weekly care for at least 12 weeks after the acute phase resolves, then tapering to whatever cadence your spine actually needs.

That sounds like a lot. It is not a sales pitch. It is what your spine needs to actually change the pattern. Each flare leaves the system slightly more guarded than before. Pretending otherwise is what leads to the chronic-pain version of this story 5 years from now.

The brand stays exactly the same. No contracts. No packages. No prepaid plans. $40 every visit. Stop coming whenever you want. The decision is yours, every visit. We just tell you the truth about what your spine needs and let you choose. Most patients who genuinely commit to the 12-week rebuild come back less often per year than patients who only show up when the disc flares.

Frequently asked questions

How do I know if my back pain is actually a herniated disc?
The classic pattern is back pain plus leg pain that follows a dermatome, often worse with flexion or prolonged sitting. The exam confirms it: a positive straight leg raise or slump test, a dermatomal sensory pattern, motor weakness in a specific muscle, or a reflex change. Imaging is not required for most cases, but it is the only way to confirm the lesion definitively.
Do I need an MRI before chiropractic care?
Usually no. For most disc-herniation cases without red flags, the exam tells us what we need to know to start conservative care. If your history or exam findings would change the treatment plan based on what imaging shows, I will refer you for it then.
Will chiropractic care make my herniation worse?
For an acutely symptomatic disc, the work is conservative: decompression-style mobilization, modified positioning, gentle traction, and avoiding loads that flare it. High-velocity manipulation is not the default; it is selected only when the exam supports it. The goal is to take pressure off the nerve, not add to it.
How long until I should expect to feel better?
Most acute disc-driven radiculopathy responds to conservative care across four to twelve weeks. Some cases respond within a few visits; others need the full window. If you are not meaningfully improved by visit 3 or 4, we broaden the plan, often with imaging or a surgical opinion.
How many visits will I need for a herniated disc?
Visit 1 is what I call the bandaid. Walk in, get an exam, get the first adjustment. If that is all you want, that is what you get, and I will hand it to you without an argument. Beyond that, there are three honest phases. Relief care is 2-3 visits per week for 2-3 weeks until the acute presentation calms. Corrective care drops to 1-2 visits per week for the next 4 to 8 weeks while supporting mechanics rebuild. Wellness care is ongoing scheduled care for months to years, regardless of how you are feeling, to prevent the next flare from ever starting. Most people stop at relief or corrective; the issue often comes back when they do. The real gold of chiropractic is wellness, and that is the patient I most want to see. Same $40 every visit. No contracts, no packages, no membership fees, and you can stop any time, but I will tell you straight up what I recommend.
When does a herniated disc need surgery?
Surgery is reserved for cases with progressive motor weakness, cauda equina syndrome, intolerable pain that does not respond to several weeks of well-applied conservative care, or specific anatomical findings that make non-operative care unlikely to work. Most herniations do not meet that bar. The screening at your first visit identifies which group you are in.
Do you take insurance for disc-herniation visits?
No. Vanguard Chiropractic is cash-pay only. Every visit is $40 flat, paid at the desk. We do not bill insurance and we do not work with personal-injury attorneys or auto-injury claims.
What if my disc problem started after a car accident?
Vanguard Chiropractic is not a personal-injury or auto-injury clinic. If your disc symptoms are part of an active PI or auto-injury claim, you will be better served by a clinic that handles that documentation. We can still see you on a cash-pay basis if you prefer that, but we will not bill any insurance or attorney.
What does a $40 disc-herniation visit actually include?
Intake, focused disc-and-radiculopathy exam, conservative care if your case is appropriate for chiropractic, and a brief home-work conversation. Same flat $40, every visit, every patient. No upsells, no packages, no sticker shock at the desk.
